UPDATES: Release Notes: The name/description/comment for CVX codes 09 and 113 have been updated to make it more apparent the difference between these two codes. The new CPT code 90756 (Influenza virus vaccine, quadrivalent (ccIIV4), derived from cell cultures, subunit, antibiotic free, 0.5mL dosage, for intramuscular use) has been mapped to CVX code 186 in the CPT-to-CVX mapping table. The NDC codes for Td (adult) by Mass Biologics are now mapped to CVX 09. Decavac is now marked as an inactive product. The status of CVX code 145 (respiratory syncytial virus monoclonal antibody (motavizumab), intramuscular) has been updated from “Pending” to “Never Active”. The status of CVX code 77 (tick-borne encephalitis vaccine) has been updated from “Inactive” to “Non-US” to reflect the use of this vaccine in Europe. Several changes have been made to account for the licensing of a second zoster vaccine: The CVX Short Description for existing CVX code 121 has been updated from “zoster” to “zoster live” to more clearly indicate that this code should be used only with the live attenuated vaccine New CVX code 187 (zoster vaccine subunit) has been create New CVX code 188 (zoster vaccine, unspecified formulation) has been created The CVX for Vaccine Group value for the ZOSTER vaccine group has been updated to CVX 188 (zoster vaccine, unspecified formulation), including updating the mapping of CVX code 121 (zoster vaccine, live) CVX codes 187 and 188 have been mapped to the ZOSTER vaccine group A new product for SHINGRIX has been created NDCs for the new vaccine have been added CPT code 90750 (Zoster (shingles) vaccine (HZV), recombinant, sub-unit, adjuvanted, for intramuscular use) has been mapped to CVX code 187 The NDCs for the 2017/2018 FluMist Quadrivalent vaccine have been added. Meningococcal polysaccharide vaccine (MPSV4) has been discontinued and all lots have expired. The status of CVX code 32 has been updated to “Inactive” and the status of the product Menomune has been updated to “Inactive”.
